13-14 Outside Scholarship Form (links to BannerWeb). Log in and click on 1) Financial Aid, 2) Award, and 3) Award for Aid Year. Pick 2013/2014 Aid Year from the drop down menu. You may submit your outside scholarships electronically to our office on the Resources/Additional Information tab.

Note: The Perkins MPN and Entrance Counseling may not be faxed or emailed. We must have the original signature on both items. Please drop the forms by, or mail them to, the Office of Financial Aid, 1845 E Northgate Dr., Irving TX 75062.
but you are classified as a Texas resident and therefore are eligible to pay the Texas in-state tuition rate, use this 2013-2014 TASFA application. To use this application, your classification as a Texas resident must not be due to a waiver, such as an in-state scholarship or an assistantship.

When completing your FAFSA online, you have the option of automatically retrieving your income and tax data from the Internal Revenue Service and having it transferred to your FAFSA. The IRS Data Retrieval Tool is the preferred method to complete the FAFSA. The IRS Data Retrieval Tool is the fastest and easiest option and will satisfy the tax transcript requirement.
If you are required to request a tax transcript from the IRS (please see the Verification Worksheet), you may do one of the following:
Paper request- IRS Form 4506T-EZ (Recommended). This option will allow you to have the tax return sent directly to
UD from the IRS. If the tax transcript is not sent directly to UD from the IRS, you
must sign the transcript before sending it to our office.
