To enroll, please print and complete the appropriate enrollment forms located to the right. Enrollment forms must be submitted to HR within 31 days from your date of hire or 31 days from the date your employment status changes to benefits eligible. Benefits are effective on the 1st of the next month after hire date.

UD offers two FSAs- medical and dependent care. The 2015 maximums for these accounts are $2,550 for the medical FSA and $5,000 for the dependent care FSA.
UD provides basic Life, AD&D, and Long -Term Disability Insurance to eligible employees working 30 or more hours per week. Employees are automatically enrolled in this free benefit and must submit a Life, AD&D, and Long-Term Disability enrollment form to HR. Employees have the option to purchase Short-Term Disability and/or increase their benefit for Voluntary Life Insurance and have the option to purchase buy-up Long-Term Disability Insurance. Please review the Life, AD&D, Long-Term Disability Plan Summary above for additional information and cost. If an employee purchases additional life insurance within the first 31 days of employment and does not exceed the Guarantee Issue, he/she is not required to provide evidence of insurability. However, if an employee purchases additional life insurance above the Guarantee Issue or after the first 31 days of employment, he/she must provide evidence of insurability to the insurance company for approval.
