SDS Accommodations Request Form

The purpose of this application is for students enrolled at the University of Dallas to request authorization for accommodations in accordance with the Americans with Disabilities Act (ADA) or Section 504 of the Rehabilitation Act of 1973.

Please fill out this application completely, and then submit the required documents to the ADA/Section 504 Coordinator at ada@udallas.edu. Once submitted, Student Disability Services will review the request and documentation to determine if the student has a qualifying condition under the ADA/Section 504. Student Disability Services will also verify that a credentialed professional substantiates the student's request and that the assessment has been conducted recently, generally within the past three years. Accordingly, Student Disability Services will maintain a student’s disability information in a confidential case management file. In order to balance student privacy with the need to provide services and accommodations, SDS will disclose student disability information only to University personnel who have a need to know that information or with a student's consent, and in accordance with the foregoing policies.
