Student Employee Information and Agreement

I-9 Form (please remember to bring your original identification document(s) when you submit this form)

   * Form I-9 instructions

   * Form I-9 form 

W-4 Form

Payroll Memo & Authorization for Deductions

Electronic Delivery of IRS Forms

Direct Deposit Form (a voided check or bank printout that confirms routing and account number is required)

Zenith Workers' Compensation (only fill out and submit page 9)
