If you would like to support the mission and vision of UD, expressed through the work of the fine folks in the UDCC, by describing how your therapist, the Case Manager, or the UDCC, in general, have supported you, please fill out the following form. We want to assure you that no personal information will be collected here, for your confidentiality. We ask that you be mindful of your confidentiality when submitting this form.
Your anonymous responses may be used in promotional materials for donors and supportive messaging for future students/parents. They may also provide our staff with the recognition they rarely receive, all of which helps guide the work of the UDCC toward our vision: Genuine care inspired by a sense of hope, healing, and resiliency through life struggles into human flourishing, leading to a more deeply enriched community.
