UD Health Form

New Students must have a completed health form on file in order to receive treatment. This 5 page form is available from the New Student Forms web page. New students that do not submit this completed form prior to arrival on campus face an account hold.   The hold will prevent class selection and ability to move into campus housing. A complete list of required vaccines is included on the dedicated Immunizations page within the form.
