Meningitis Vaccination Details

Satish & Yasmin Gupta College of Business Bacterial Meningitis Vaccination Record

Effective January 1, 2012, Texas House Bill SB1107 requires entering students to provide evidence of vaccination against bacterial meningitis during the five-year period preceding, or at least 10 days prior to, the first day of the first term.

Entering student is defined by the Texas Higher Education Coordination Board as:

  • a new or transfer student enrolling at the institution for the first time
  • a returning student who was enrolled at the institution prior to January 1, 2012, and is returning to enroll following a break in enrollment of at least one fall or one spring term.

The following are exempt from the immunization requirement:

  • any student 22 years of age or older on the first day of the term
  • any student enrolled in only online courses, distance education courses, or continuing education corporate training courses

 

WAIVER OF IMMUNIZATION

Students who wish to be exempt from the vaccination may submit:

  • A letter signed by a physician who is duly registered and licensed to practice medicine in the United States, in which it is stated that, in the physician's opinion, the vaccination required would be injurious to the health and well being of the student. Please submit this form to your doctor for signature: Medical Exemption Form (PDF)
  • An affidavit signed by the student stating that the student declines the vaccination for bacterial meningitis for reasons of conscience, including a religious belief.

Students residing in campus housing must submit a conscientious exemption form from the Texas Department of State Health Services, which may be obtained here.

  • Form takes approximately two weeks to receive from the state
  • Students over the age of 18 may complete the form by listing their name as both the parent and student
  • Student must submit original notarized affidavit

Students living off-campus may print the affidavit form from the Texas Higher Education Coordinating Board, which may be obtained here.

  • Students over the age of 18 may complete the form
  • Student must submit the original notarized affidavit

ACCEPTABLE VERIFICATION OF IMMUNIZATION

  • A document bearing the signature or stamp of the physician or his/her designee, or public health personnel (must include the month, day, and year the vaccination was administered).

  • An official immunization record generated from a state or local health authority (must include the month, day, and year the vaccination was administered).

  • An official record received from school officials, including a record from another state (must include the month, day, and year the vaccination was administered).

IMMUNIZATION LOCATIONS

Students may request the meningitis vaccination at the UD Health Center by calling 972-721-5322 or emailing Dr. Lora Rodriguez. The cost of the meningitis vaccination is $150. Please visit the UD Health Center website for additional details.
