The online health form is required of all new incoming undergraduates, including seminarians, for enrollment and to receive a housing assignment.  Student Health Clinic staff will review each student's online health form and advise about any additional information required.  Students must have a reviewed and complete online health form on file in order to receive treatment at the Student Health Clinic.  This form is available from the New Student Form web page.

New students who do not complete all requirements of the online health form by the deadline will face an account hold.   The hold will prevent class selection and ability to move into campus housing.
