End of Year Form

Please fill this out at the end of each Spring Semester or if you will not be returning to campus. If you do not keep us up-to-date on this site, your UD box will expire in June and be re-assigned to other students. In that case, we will forward first class mail if possible and return to sender or dispose what remains.
